About the Organization ASHwell is a nonprofit, sexual health clinic and social wellness center established in Austin, TX in the late 1980s. Clinically, we specialize in HIV treatment and prevention, Hep C treatment and STI treatment and management. Socially, we specialize in providing programming, resources and services to overcome barriers to health access with the goal of building Health Equity in Austin and Central Texas. ASHwell prioritizes providing a safe, welcoming and discrimination-free space for all, predominantly serving LGBTQIA+ and BIPOC communities. Position Overview The Social Wellness Case Manager will provide comprehensive social service coordination and individualized support for ASHwell’s patients and partner-referred clients. Their work measurably improves health outcomes through culturally competent and trauma informed case management and resource navigation. In addition to general case management, the case manager may specialize in working with clients who require high competency in the categories of: Expansive Gender & Sexuality, Language and Accessibility, or Crisis Support if their experience and skillset aligns. The Case Manager collaborates with an internal multidisciplinary team and external partners to ensure comprehensive care that aligns with the organization’s mission and values. They help coordinate care, provide navigation for social support services, educate on social resource management, and act as advocates in social service and public system settings. They maintain consistent and reliable communication to build a relationship of trust. The Social Wellness Case Manager enters, tracks and reports data regularly and manages their time effectively.

Our Case Manager understands the intersecting needs of the people ASHwell serves and can intuit what resources, services and programs will help reduce and eradicate their barriers to staying in care. They can connect their role and its responsibilities to the bigger picture of ASHwell’s mission.

TIMELINE
ASHwell uses a cohort onboarding model. The start date kicks off three days of group orientation for hires across the organization.
